Beacon Health System is hiring a Care Coordinator (RN, BSN required) for our Physiatry Department in South Bend, IN.

What You’ll Do:

• Coordinate care for patients across hospital, emergency, and outpatient settings
• Support patients and families through admissions and discharges
• Collaborate with physicians, nurses, and specialists to ensure seamless transitions
• Contribute to quality improvement and patient-centered initiatives

What You Bring:

• BSN & active Indiana RN license
• 3+ years of clinical experience
• Strong communication, collaboration, and problem-solving skills
