APR Consulting, Inc. has been engaged to identify a Health Information Specialist I

Location: Los Altos, CA 94022-1442 - 100% on site

Position: Health Information Specialist I

Pay Rate: $32.98/hr

Duration: 13 weeks

Expected Shift: Day 5x8-Hour (08:00 - 16:30)

Float between - MPMC (Burlingame), PAMF (Los Altos/Santa Cruz)

JOB ACCOUNTABILITIES:

Assignment Specific Responsibilities

May perform one or more of the following functions:

  • Ensures routine requests for medical records, including subpoenas, are processed within operational guidelines for timeliness, following all applicate policies and procedures, state and federal law.
  • Prepares, scans, indexed, and performs quality assurance review of patient documents in the electronic health record. Identifies and reports use of unapproved forms. Provides appropriate education to departments that perform point-of-service scanning.
  • Locates and tracks legacy records as appropriate.
  • Coordinates merges of medical records across systems and physical charts, ensuring completeness and accuracy, involving upkeep of the Enterprise Master Patient Index (EMPI), following all policies and procedures.

  • Consistently communicates with individuals with respect, kindness and understanding. Is honest and clear, treats sensitive information confidentially, is perceived as positive and demonstrates quality services.
  • Maintains personal contact with manager(s) to ensure understanding of all assignments, the goals of the team and of the organization. Participates in individual and section meetings.

Performance Management

  • Ensures delivery of business results by meeting or exceeding all individual operating metrics.
  • Plans and organizes work so Individual Operating Metrics and Service Level Agreement objectives are realized.
  • Recognizes and communicates potential issues to his/her team leader as appropriate.
  • Consistently takes responsibility for learning knowledge and skills required to perform own job responsibilities effectively and apprising self of relevant policies and processes.
  • Enhances professional growth and development through participation in educational programs and in-service meetings, as appropriate.
  • Continually uses insight and feedback from others to assess own performance, identify gaps between desired and actual performance, and improve own performance.

EDUCATION:

  • Equivalent experience will be accepted in lieu of the required degree or diploma.
  • HS Diploma or equivalent education/experience

TYPICAL EXPERIENCE:

2 years recent relevant experience

SKILLS AND KNOWLEDGE:

  • Knowledge of The Joint Commission (TJC) standards, California Medical Information Act (CMIA), Centers for Medicare and Medicaid Conditions of Participation (COP), Title 22, AAAHC (ASC), IMQ, and other state and federal laws as appropriate.
  • Knowledge of changes in laws relating to release of information by regular attendance of Regional, State and National seminars.
  • Basic knowledge of anatomy and physiology.
  • Familiarity with HIM within the Revenue Cycle.
  • General knowledge of all areas related to Document Imaging/Forms Management and how they interrelate.
  • Ability to communicate ideas both verbally and in writing to influence others using one-on-one contact and group discussions.
