Job Description

Pride Health is hiring for a Home Health RN for one of our clients located in the Charleston area! (Contract)

Coverage: Berkeley, Dorchester, and Charleston Counties

Schedule: Monday-Friday (days), on call if requested

Contract: 3 months

Pay Rate: 40-42/hour

Essential Functions:

• Assess home health/hospice patients in routine and emergent situations to identify physical, psychosocial, and environmental needs.
• Develop, implement, document, and evaluate the Plan of Care to ensure quality and continuity.
• Gather and incorporate physical, psychological, social, spiritual, and cultural data into care planning.
• Ensure continuous quality care with accurate, up-to-date documentation.
• Provide care following OSHA infection control standards.
• Educate patients and families on care, medications, safety, and self-care using appropriate teaching methods.
• Supervise and guide HHA/STNA/LVN staff to ensure quality services.
• Assess and manage pain and educate patients and families.
• Arrange referrals to appropriate agencies as needed.
• Monitor cases for third-party payer compliance.
• Perform assigned on-call duties.
• Communicate effectively and act as liaison with patients, families, and healthcare team.
• Attend interdisciplinary and required meetings.

Requirements

• Homecare Homebase Software Experience - Required
• OASIS Experience - Required
• Current state license as a Registered Nurse - Required
• CHPN certification - Preferred
• Proof of current CPR certification - Required
• Current driver's license - Required
• Must have reliable transportation and insurance - Required
• 1-2 years experience as a Registered Nurse in a clinical care setting - Required
• 2+ years Home health experience - Required.
