Pay Range: $40 - $43/hour. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).

Note-Location: Field. Regions 2 and 3

Key Responsibilities:

• Care Managers will provide specialty service care management to beneficiaries. Will collaborate with Care Managers (CM), Care Coordinators (CC), Transitional Care Managers (TCM(, and Providers/Agencies to ensure appropriate services are delivered to ensure continuity of care across services.
• Responsible for the total care plan, encompassing all services and plans related to beneficiaries. Provide case management complying with Conflict Free Case Management rules.
• Provide care management to members by reviewing member charts, coordinating with LTSS Care Manager face to face/home visits (F2F/HV), and checking calendars/schedules for visits.
• Care manager will visit member in home, hospital, assisted living facility, and/or member’s setting of care.
• Position will consist of visits by traveling up to 2 ½ hours in assigned region(s) of member.
• This job will consist of travel of at minimum 90% on a daily basis to conduct home visits for members but can vary per week depending on associates ability to document in home and/or collaboration with LTSS CMs.
• If unable to document in the members home, CM will document at regional satellite and/or secure remote office.

Key Requirements and Technology Experience:

• Must have an active NC RN license.
• Care management experience.
• Reliable transportation and active DL.
