We are seeking a Nurse Practitioner to join our team and support our Health Center Physicians in delivering exceptional patient care in San Jose, CA. The Nurse Practitioner delivers comprehensive medical care to patients at the Health Center, with a focus on primary care experience. This role involves assessing, diagnosing, and treating patients, prescribing and dispensing medications, educating patients about illnesses, diseases, and treatments, documenting care, and maintaining clinical records.

Education/Experience:
Master's Degree in Nursing
Minimum of two years of Primary Care experience.
Active, unrestricted license with the California Board of Registered Nursing as a Nurse Practitioner.
Duties and Responsibilities:
Provide continuing, comprehensive healthcare maintenance and medical care to patients, including preventive, behavioral, and community health care.
Provide accurate assessment, diagnosis, and treatment based on the ordered medical history, physical examination, and diagnostic tests.
Prescribe and dispense medications.
Develop and implement treatment plans with patients and other care team members.
Educate patients on topics such as disease process, medication, prevention, and compliance.
Refer patients to affiliated subspecialists when indicated.
Document and maintain clinical records per appropriate standards and regulations.
Participate in on-call after hours rotation.
Provide coverage for other clinicians when needed.
Participate in clinical quality improvement processes, including peer reviews.
Maintain certification, professional training, and continued medical education.
Participate in team meetings, data collection, health outcomes reporting, clinical audits, and program evaluation.
Complete other related duties and projects as assigned.
