Transitions of Care pharmacy technician position with a primary responsibility of performing medication histories with patients in the Emergency Department. The process includes interviewing patients and family members and utilizing online prescription resources to compile the best possible medication list for patients being admitted to the hospital. Excellent communication and problem solving skills as well as prior hospital pharmacy experience are keys to success with this position. Previous Epic use is also beneficial.

The Pharmacy Transitions of Care Technician, under the direct supervision of a pharmacist, will assist the care team through various phases of the patient’s care including but not limited to: 1) Gathering the best possible medication history that is then utilized by the care team for medication reconciliation, and 2) Facilitates the provision of medications at the time of discharge from the hospital.

Education:

• High school diploma or equivalent required.
• Graduate of a pharmacy technician program preferred.

Experience:

• Minimum 2 years of experience as a pharmacy technician required; retail and/or inpatient hospital experience preferred.
• Experience applying customer service behaviors and communication skills required.
• Basic familiarity with computers and ke yboard skills and ability to navigate electronic systems applicable to job functions.
