Job Description

We are seeking a dedicated Physical Therapist to join our team and provide exceptional restorative and rehabilitative care to participants both in our center and at home. Working alongside a multidisciplinary team, including Primary Care Physicians and rehabilitation specialists, you will play a vital role in maximizing patient independence, safety, and overall well-being.

Key Responsibilities:

✔ Conduct comprehensive Physical Therapy assessments in facility and home settings

✔ Collaborate with the IDT team to develop personalized care plans

✔ Provide and document therapy interventions per physician orders and care plans

✔ Perform active and passive exercises, muscle re-education, gait training, and ADL support

✔ Oversee Therapy Assistants, providing training and guidance for Functional Maintenance Programs (FMP)

✔ Educate staff, participants, and caregivers on therapeutic equipment use and care

Qualifications:

🎓 Education:

• Bachelor’s Degree in Physical Therapy + 5 years of experience required
• Master’s or Doctorate in Physical Therapy preferred

📜 Certifications & Licenses:

• Valid California Physical Therapy License (or proof of eligibility)
• CPR certification preferred

🩺 Experience:

• 2+ years in a clinical setting with elderly or frail populations preferred
• International degrees must be evaluated for U.S. equivalency
