Flexible hours, mileage reimbursement, growth structure available (Make your own schedule ) The LPN or RN will conduct the initial assessment, re-assessment and discharge assessments for our active clients. The nurse for this position will be responsible for entering the clients information into our portal. In addition to completing the assessments, the nurse will also be ensure that all documents and the clients file are audit ready at all times. Our success as team, involves a dedicate nurse that can handle the monthly schedule of assessments that are due. The nurse joining the team is a vital part of our growth and success. The nurse joining our team will receive the training and support throughout their career here at AHC. Responsibilities:

• The nurse for this position will be responsible for entering the clients information into our portal
• The nurse will also be ensure that all documents and the clients file are audit ready at all times.
• Prepare and schedule monthly re-assessments
• Assist with marketing events
• Complete initial assessments, re-assessments, and discharge assessments
• Provide medication education to clients and field employees

• Must have good standing with state of GA Nurse Registry
• Current TB or chest X-ray
• First Aide and CPR
• Reliable transporation ( ride share not acceptable form of transportation)
