Job Overview:
This position provides services to clients of the Community Health Access and Navigation in Tennessee (CHANT) model under the direct supervision of the CHANT Team Lead. The CHANT care coordinator (social counselor) position conducts care coordination services in the home for target populations, including pregnant and postpartum adolescents and women, all children up to 5 years old, TennCare Kids eligible children birth to 20 years old, and children and youth with special healthcare needs birth to 21 years old. CHANT serves all family members within the home. In this position you will work closely with local health department staff and community partners to ensure the family’s needs are met. The purpose of CHANT is to identify and address risk factors and provide resources to individuals and families.
Key Responsibilities:
• Participate in daily morning huddles. Receive assigned referrals from CHANT Team Lead.
• Contact referred family for initial engagement per CHANT guidelines.
• Conduct a screening and assessment with individuals/families.
• Assist the family in navigation of care and goal setting per pathways of care.
• Identify appropriate referrals/resources based on the child/family needs.
• Educate family using key educational messages.
• Plan, conduct, and participate in targeted community-based activities.
• Document CHANT efforts completely and accurately in the EHR system
• Participate in CHANT related continuing education activities.
• Follow CSS policy for children eligible for medical service reimbursement.
Minimum Education and Experience Requirements:
Graduation from an accredited college or university with a bachelor’s degree AND one year of professional experience in one or a combination of the following: social work; or social or psychological counseling, including, but not limited to, mental health, correctional, or behavioral rehabilitation counseling; or one year of experience in early intervention and/or service coordination with individuals with disabilities in an educational, social services or medical setting.
Graduate Course Work for Experience: Additional qualifying graduate course work from an accredited college or university in a related field may be substituted for the experience, on a year-for-year basis, to a maximum of one year.

• Attend in-service trainings and other meetings as assigned.
• Basic computer skill or demonstrate willingness to learn job related computer programs.
• Must have valid driver’s license and reliable transportation.
• Good communication skills
Monthly Salary:
Minimum: $3,542 / month
