Job Title: Weekend Occupational Therapy Assistant – In-Home Care Model

Company: Home Health Care, Inc.

Schedule: Weekend Visits (Flexible Every/Every Other Weekend)

Pay: $42 - $46 per visit.

Mileage reimbursement between clients!

Duties

• Facilitate daily living skills, mobility routines, and therapy-based activities under OT direction.
  
• Provide coaching and home exercise instruction to clients and caregivers.
  
• Record all visit data and patient insights to aid the clinical team’s planning.
  
• Collaborate with other disciplines for integrated care.
  

Skills

• Current Minnesota COTA license.
  
• Passion for home-based care.
  
• Excellent communication and adaptability.
  
• EMR experience preferred.
